Unless the creature is actively covering the trail (and even then) they'll leave grass slightly squished, leaves and branches ruffled or broke, footprints in the dirt, etc.. They leave their traces and their marks as they warp reality to arrive at their destination, and just like a sufficiently skilled survivalist can follow a trail, a sufficiently skilled magic user is capable of following a teleport. Aximiam is said caster. He's managed to pinpoint the location of the teleport spell using that tracking metaphor and takes the necessary precautions before teleporting to new coordinates.
